The Baystate Health Foundation has established The Baystate Health Legacy Society to give special recognition to those generous individuals and families who have made provisions in their will or estate plan to support the hospital and its mission and have informed us of their intentions during their lifetime. Enrolling you in this society also gives us a chance to thank you during your lifetime for having the vision to establish a generous planned gift that will benefit the hospital and the communities it serves.
Members and their guest(s) will be invited to attend special donor recognition events as our guest. Unless you request that your intentions remain anonymous to the public you will be recognized during your lifetime in an annual list of Society members to be published in our foundation Annual Report of donors and on our health system web site. Members’ names will also be engraved onto a special glass sculpture on display on the first floor of our hospital in Springfield.
Membership in The Baystate Health Legacy Society will be offered to those who have included a gift to the Baystate Health Foundation or its affiliated hospital programs for the benefit of our health system in their estate plan. The planned gift could be in the form of a bequest intention, charitable trust, insurance plan gift, charitable gift annuity, retirement account beneficiary, planned real estate gift, or other estate planning vehicle. A simple statement of intent on this form and a copy of any pertinent language on official documents pertaining to the gift on file in our Baystate Health Foundation office will enroll you in our Legacy Society.
